What is Support Coordination?

Support Coordination is a NDIS funded support that is sometimes included in a plan to assist you to develop your understanding of your NDIS plan, to assist you to engage supports tailored to your specific needs, to assist you to resolve points of crisis and develop capacity and resilience, and to assist you to prepare for your plan review.

It’s important to understand not everyone does get support coordination, and it depends on a variety of factors.

You’ve been approved Support Coordination, what now?

At Sage, we like to do things a bit differently, and put you at the centre of everything we do. By understanding your needs thoroughly, we can help you to achieve the best outcomes, and assist you to achieve your plan goals. This meeting will give us an opportunity to discuss what you’d like out of your plan funding, and to work together to set clear expectations about which roles we fulfil and which we can provide limited or no support in. At Sage, we believe in being upfront and transparent, so we build respectful and honest communication.

Our initial meeting is spent getting to know you, reading through your plan, and to discuss your needs and individual situation. By taking the time to get to know you, learning about your family and through active listening and developed experience – we can ensure you are genuinely heard, that not only your disability needs are supported, but you are seen for who you are – and we assist you to get supports that not only assist you to engage in everyday life, but are meaningful and genuinely supportive.

I’ve only got a limited amount of Support Coordination, what happens when I’ve used it all?

Support Coordination is built into a plan to assist you to develop your capacity. Sometimes it may not be possible, and we understand that too. Sage is here to assist, whether that be for a single plan or for support over many years.

At Sage, we work with you – to set your plan up and provide ongoing support for the duration of your funding. If Support Coordination funding is exhausted – we have a variety of options, depending on your individual needs. This may include asking for a review, accessing other areas of funding under COVID measures, or we may work collaboratively with your Planner or LAC to make sure you stay supported.

There are also avenues we can take if you would like to engage Sage, have a chat with us about your specific needs.

Does Sage service only some people? Will they understand my disability, and care about who cares for me?

The Support Coordinators at Sage have a diverse and wide base of knowledge and experience, and work with a variety people of different backgrounds, with a variety of needs, and varying situations.

We can work with people with all disabilities, at all life stages, and are respectful and inclusive to people of all backgrounds, cultures and sexual identity. At Sage, we welcome you, and will treat you with the utmost respect and kindness.

We have extensive experience across a range of disabilities and demographics, and are yet to meet a participant we couldn’t help.

What is Specialist Support Coordination?

Specialist Support Coordination is a higher level of support than Support Coordination. It focuses on reducing complexities within a participants support environment and helping participants overcome barriers in accessing their NDIS services.

Specialist Support Coordination is designed for participants who are a faced with crisis, or, have complex needs and receive support from other sectors, including justice, youth services, legal, health, and mental health services. Specialist Support Coordinators will work with these services to support you to achieve the outcomes you want and progress your NDIS goals.

Our team can facilitate both Support Coordination and Specialist Support Coordination, depending on what’s right for you and your situation.
